"Implant surgery triggered" Bisphosphonate-Related Osteonecrosis of the Jaws (BRONJ) : Case report

Abstract
Bisphosphonate-related osteonecrosis of the jaw (BRONJ) is a rare, but serious, side effect of bisphosphonate therapy
that produces significant morbidity in affected patients. It is characterized by poor wound healing and spontaneous intraoral soft tissue breakdown, which lead to exposure of necrotic maxillary and mandibular bone. In the last years, bisphosphonate-related osteonecrosis of the jaws(BRONJ) has been carefully studied with particular interest to the etiology. "Implant surgery triggered" BRONJ are uncommon. But the influencing factors and types of bone destruction are various, and the surgeons should well-acquainted of all known independent risk factors, those prognosis as well. This report documents a case of bisphosphonate-related osteonecrosis of the jaw (BRONJ) after dental implant placement in an osteoporotic patient treated with alendronate for 4 years. We discontinued alendronate and started conservative care, and curettage and sequestrectomy were performed under general anesthesia. During 3 month followup period, the patient showing a good prognosis up to date. From this paper, we report a case of oral bisphosphonaterelated osteonecrosis of the jaw (BRONJ) resulting in implant failure. Also, we want to know the proper treatment strategies on the basis of a review of the literature.
